Detecting differences with magnetoencephalography of somatosensory processing after tactile and electrical stimuli.

Abstract Background Deviant stimuli within a standard, frequent stimulus train induce a cortical somatosensory mismatch response (SMMR). The SMMR reflects the brain’s automatic mechanism for the detection of change in a somatosensory domain. It is usually elicited by electrical stimulation, which activates nerve fibers and receptors in superficial and deep skin layers, whereas tactile stimulation is closer to natural stimulation and activates uniform fiber types. Ultrasound-Guided Percutaneous Neuromodulation in Patients with Chronic Lateral Epicondylalgia: A Pilot Randomized Clinical Trial

Objective: The aim was to analyze effects of a percutaneous neuromodulation (PNM) treatment on the radial nerve, regarding pain, functionality, electrophysiologic excitability, and morphology, in patients with chronic lateral epicondylalgia (LE). Methods: Twenty-four patients with chronic unilateral elbow pain were recruited for this preliminary study and were divided into two groups: control (n = 12) and PNM group (n = 12). The subjects in the PNM group received percutaneous peripheral neurostimulation with an acupuncture needle that was located next to the nerve with ultrasound guidance. About 2% of the world’s population suffers from small nerve fiber dysfunction, neuropathy, which can result in severe pain. This condition is caused by damage to the small nerve fibers and its assessment is challenging, due to the lack of simple and objective diagnostic techniques. Background and aims Neuroaxial anaesthesia is gold standard for lower extremity surgeries. Nowadays the introduction of ultrasound guided peripheral nerves blocks (PBN) changed the approach to orthopaedic surgery. Methods This case series evaluates postoperative pain in 17 patients with femoral or knee fractures who received PNB because of contraindications to subarachnoid anaesthesia (eg. anticoagulation therapies, refusal).
